CLAY COUNTY — Ahead of the new school year, the Florida Department of Health in Clay County will offer free immunization services at the Clay County Fairgrounds from 8 a.m. to 2 p.m. on Aug. 14.
Although services are free, there will be a $15 fee for any child who needs an updated DH Form 680 without immunizations.
Each child must accompany a parent/guardian and provide current immunization records.
Additionally, DOH-Clay is offering infant and adult immunizations and school-entry examinations by appointment only at their clinic. For more information, call (904) 529-2800.
