Arrest made for insurance fraud
ORANGE PARK – An Orange Park area woman faces various charges of fraud following a lengthy investigation from the State of Florida Department of Financial Services.
The Clay County Sheriff’s Office arrested Gwen Villagomez Soto, 43, on March 29 using a warrant the state obtained shortly after interviewing Soto on March 14.
According to the warrant, the case began on November 6, 2015 when Soto – who is also known as Gwen Mixner – filed a disability claim with Metlife. Soto, who was employed by Citi at the time, requested special accommodations due to her condition. However, according to the warrant, “Mixner failed to respond with the required Accommodation Request Form…paperwork and the claim was closed on 9/7/2017.”
The investigation found that, six days later, Mixner sent the accommodation form to Metlife, who then reopened her disability claim. In the midst of processing the claim, Metlife contacted Mixner’s physician only to find she had forged his name. By the end of September 2017, the case was turned over to the state’s fraud unit at the Department of Financial Services.
“This investigation revealed that Gwen Mixner committed insurance fraud when she knowingly provided false and misleading information that was material to a claim,” states the warrant.
Soto faces one count each of forgery of a doctor’s certificate and filing false and fraudulent insurance claims. She has since been released from the Clay County Jail on $10,006 bond.
